OWNER INFORMATION
Financial institutions are required to identify all individuals who, directly or indirectly, own 25% or more of the equity interest of the business. Therefore, you will need the following information for all owners who own 25% or more of the business' equity, even if they will not be signers on the account.
- Full Legal Name
- Percentage of Ownership
- Social Security Number
- Date of Birth
- Valid Driver’s License Number/ID with an Issue and Expiration Dates
- Phone Number
- Email Address

SOLE PROPRIETORSHIPS WITH A FICTITIOUS NAME
- Federal Employer Identification Number (EIN) or Social Security Number of Sole Proprietor
- Fictitious name verified with the State of Florida’s Division of Corporations website (www.sunbiz.org) unless the following:
You are using your full legal name in the Business Name.
For example, if your name is Eugene Smith and your business
name is Eugene Smith, you won't need to register a fictitious
name. However, if the business name is Eugene Smith Jewelers,
you will need to register a fictitious name.
PARTNERSHIP ACCOUNTS
- Federal Employer Identification Number (EIN)
- Partnership Registration verified with the State of Florida’s Division of Corporations website (www.sunbiz.org)
- Copy of entire executed Partnership Agreement
- All Signers/Beneficial Owner Information
CORPORATIONS (S-CORP, C-CORP, LLC) INCLUDING ASSOCIATIONS/ORGANIZATIONS
- Federal Employer Identification Number (EIN)
- Articles of Incorporation (S-Corp/C-Corp) or Articles of Organization/LLC Agreement (LLC) Registered with the State of Florida Corporation verified with the State of Florida’s Division of Corporations website (www.sunbiz.org)
- All Signers/Beneficial Owner Information
As part of your account opening process, we will also present you with a Resolution of Authority Form and Business Service Account Agreement.
